Responsibility of School-Issued Curriculum

Please read through the agreements regarding the use of iLEAD Hybrid Acton Studio’s curriculum and materials:

1. I acknowledge that all materials and curriculum provided to me remain iLEAD property.
2. I agree to report any missing items within a 48-hour window if I do not conduct a joint review of the itemized sheet with staff. Failure to do so means I accept financial liability for all listed property and must either provide an exact replacement or pay the full value of the items upon my departure or at the conclusion of the academic year.
3. To prevent damage, I will keep all iLEAD AV Exploration materials in a secure location, away from food, dirt, pets, and young children. I accept financial responsibility for the replacement of any items that are damaged while in my care.

One form must be completed per enrolled learner.
